Your first visit to us will include a comprehensive assessment so that we can best meet your needs. After the assessment, you and our professional staff will work together to create a treatment plan that is right for you.

Planning For Your First Visit

On the day of your first visit, there are a few things you can have ready to make the check-in experience run more smoothly. So, here’s what to bring to ensure you feel fully prepared when you arrive for your first visit.

Insurance & Financial Information

Life Management Center is committed to making treatment as affordable as possible. For individuals without insurance coverage, a sliding fee scale is utilized for services, based on family size and income. No one will be denied access to services due to inability to pay.

To ensure equal access and an opportunity to benefit from services, appropriate auxiliary aids and services will be provided to qualified persons with disabilities at no cost to the client or companion.

Help is Available Now

We’re available 24 hours a day, seven days a week, to provide a no-cost mental health assessment for you or your loved one. Call us at 850-522-4485 or visit our facility to get started. For questions about our programs, call us.

In the case of a medical emergency or crisis, please dial 911 or go to the nearest emergency room.
